Would you understand your patients?

Two patients, two voices. Listen, answer one question – and find out whether you can even follow a dialect.

Your question

Wann ist die Patientin gestürzt?

When did the patient fall?

Answer options
Show solution

Solution: C

  • AClassic trap: “halb zehn” means 9:30 or 21:30 – not 22:30. In English, “half ten” means 10:30, which is exactly why it is confusing.
  • BClose: the time fits, but she says “gestern Abend” (yesterday evening) – so 21:30.
  • CCorrect: “gestern Abend gegen halb zehn” means yesterday at around 21:30.
  • DToo early: “halb zehn” in the evening is 21:30.

Ich bin gestern Abend gegen halb zehn in meiner Küche ausgerutscht.

Aus dem undichten Geschirrspüler war Wasser auf die Fliesen gelaufen, und weil nur das kleine Licht an war, habe ich die Pfütze nicht gesehen.

Ich bin nach vorne gefallen, habe mich mit der rechten Hand abgestützt und gleichzeitig mit der rechten Brustkorbseite gegen die Kante der Arbeitsplatte gestoßen.

Do you think you can understand a dialect, too?

Many patients in Germany speak a dialect. Test yourself with this farmer from Bavaria.

Take the Bavarian challenge

Would you like to hear it again, more slowly?

“Halb zehn” is a classic trap – and details like this have to be right in the patient history.

Or are you ready for a patient who speaks Bavarian?

Bonus challenge: Bavarian

Can you understand this farmer? He speaks Bavarian, like many patients in southern Germany.

Your question

Wo hat der Patient Schmerzen?

Where is the patient in pain?

Answer options
Show solution

Solution: B

  • AHe did land on his back (“Auf'm Ruckn”) – but the pain is in his head: “Aaah, mei Schädl …”
  • BExactly: “mitm Kopf auf an Stoa” means “with my head on a stone”, and “mei Schädl” means “my head”.
  • CThat was the first patient: she caught herself with her right hand. The farmer fell head first onto a stone.
  • DHe never mentions his leg: he fell on his back, with his head on a stone. He only mentions the cows (“d'Kiah”) because they need milking.

And what does he want? A pill – and then home, because the cows need milking. After a fall onto the head, that is not enough: in the training you practise explaining calmly and clearly why an examination matters.

Your path through the FSP

Three exam situations. One training path.

The training follows the usual structure of the Fachsprachprüfung: talk to the patient, document, then present the case to a medical colleague. The exact rules are set by the state medical chamber (Landesärztekammer) responsible for you.

01 · Patient conversation

Doctor–patient conversation

Listen carefully, capture what matters and explain things clearly.

  • Patient details: note names, date of birth, height, weight and phone numbers reliably
  • Reactions: respond with empathy to questions, worries and personal remarks
  • Emergencies: calmly pause the history-taking and explain the next steps
  • Informed consent: explain examinations and procedures in plain language

Example task

Hören Sie die Patientendaten und notieren Sie Namen, Geburtsdatum, Größe und Gewicht.

A doctor listens attentively to a patient across a desk.

02 · Arztbrief

Writing the Arztbrief

Turn colloquial notes into a structured medical letter in 20 minutes – with a focus on Konjunktiv I (reported speech).

  • Writing against the clock, like in the exam
  • Turn everyday language into medical terminology
  • Compare with a model letter

Example task

„Mir ist seit gestern übel, ich musste zweimal brechen.“

Die Patientin gab an, sie leide seit dem Vortag unter Nausea und habe zweimal erbrochen.

A doctor concentrates on writing a medical letter.

03 · Doctor-to-doctor

Discussing cases with colleagues

Present the case in a structured way and discuss it with colleagues using medical terminology.

  • Case presentation: complaints, medical history, family and social history
  • Medications: understand drug names, explain the drug class and how it is used
  • Medical terms: translate everyday expressions into medical terminology
  • Occupations: explain jobs, tasks and workplaces

Example task

Stellen Sie die Patientin in drei Abschnitten vor: aktuelle Beschwerden, Vorgeschichte, Familien- und Sozialanamnese.

Two doctors discuss a patient chart together.

Medical terminology

From everyday word to medical term

With patients you use everyday German; in the Arztbrief and with colleagues you use medical terminology. Tap a card to switch.

How it works

Practise in four steps

  1. Choose a task

Pick an area or follow the training path. A session has 10 short tasks.

  2. Answer in German

Write or speak your answer, depending on the exercise.

  3. Compare and get feedback

Depending on the task: automatic checking, a model answer for self-review, or AI-supported assessment where available.

  4. Review what is due

What you found difficult comes back at the right time – with a reminder. Your study plan keeps your exam date in view.

AI-supported assessment is not available in every exercise and can contain mistakes.

Why it works

Short, focused, plannable.

  • Short sessions

10 tasks per session – fits even a busy shift schedule.

  • Speak and write

Many exercises can be answered in writing or out loud.

  • Targeted terminology

The medical terms and phrases that count in the exam.

  • Scheduled reviews

Due tasks are planned automatically – with reminders.

  • Study plan for your exam date

Enter your exam date and study goal, and see your progress in a calendar.

Transparent

What you can expect – and what you can't

  • Fictional practice cases

All cases are made up for language training – not for medical decisions.

  • Your recordings stay private

Your voice recordings are private and not accessible to other users. They are deleted automatically after 48 hours by default, and you can delete them yourself at any time.

  • Preparation, not the exam

FSPready does not replace the official exam and does not guarantee a pass. Your state medical chamber sets the details.

  • Honestly labelled

Where AI helps, we say so. Examples are marked as examples.

FAQ

Frequently asked questions

Who is FSPready for?

For doctors trained abroad who are preparing for the Fachsprachprüfung – especially if your general German is already good (around B2) and you want to communicate confidently in medical situations. It also suits anyone retaking the exam with little time to study.

Is the exam in German – and the training too?

Yes. The Fachsprachprüfung is held in German, so the whole app – exercises, answers, feedback and interface – is in German only. Only this page is available in German, English, Arabic and Ukrainian.

Do I need a microphone?

Not necessarily. Many exercises can be answered in writing. For speaking exercises you use your phone's or computer's microphone directly in the browser.

How is the feedback produced?

It depends on the exercise: patient details are checked automatically, for other tasks you compare your answer with a model answer, and where available there is AI-supported assessment. AI can make mistakes – use it as a training aid.

What is free?

Sign-up and training are free. AI-supported assessments are limited to a certain number per month on the free plan.

What happens to my voice recordings?

Your recordings are private. They are deleted automatically after 48 hours by default, and you can delete them yourself at any time.
